Codependency Self-Evaluation

Directions: Put a checkmark next to each statement that is
generally true for you.

Interpreting Results:

Any check marks CAN indicate codependency issues.
The number of statements which you found to apply to you, as
well as the intensity of your response (that is, when you
say the statement is true, how
often is it true) gives an indication of how
much of a problem codependency may be
in your life.


If you have more than three statements checked as true,
you are not likely to be taking very good care of your most
important asset: yourself!



  • I assume responsibility for the feelings and behaviors of others.
  • I have difficulty identifying my own feelings: am I sad, lonely, happy, etc.
  • I fear and worry about how others might respond to my feelings.
  • I am afraid of being hurt and/or rejected by others.
  • I am perfectionistic, placing too many expectations upon myself and upon others.
  • I tend to put other people's wants and needs ahead of mine.
  • I tend to judge everything I do or say harshly. Nothing I do, say, or think seems to me to be "good enough."
  • I am steadfastly loyal: even when the loyalty is unjustified.
  • I have to be "needed" in order to have a relationship with others.
  • I tend not to ask for help or share. I do not think I should bother people with my problems.
