Adult ADHD : Basic List for Self-evaluation.

ADHD is difficult to diagnose. It is important that you have a professional evaluation by a medical doctor if you think this is an issue for you or someone you love.

To self-evaluate, please answer the questions below. Rate yourself on each of the criteria shown using the following scale:

          0 = never
          1 = rarely
          2 = sometimes
          3 = often
          4 = nearly always

As you answer each question, mark how you have felt and conducted yourself over the past 6 months.

_____1. How often do you have trouble wrapping up the final details of a project, once the challenging parts have been done?

_____2. How often do you have difficulty getting things in order when you have to do a task that requires organization?

_____3. How often do you have problems remembering appointments or obligations?

_____4. How often do you fidget or squirm with your hands or feet when you have to sit down for a long time?

_____5. How often do you feel overly active and compelled to do things, like you were driven by a motor?

_____6. How often do you make careless mistakes when you have to work on a boring or difficult project?

_____8. How often do you have difficulty keeping your attention when you are doing boring or repetitive work?

_____9. How often do you have difficulty concentrating on what people say to you, even when they are speaking to you directly?

_____10. How often do you misplace or have difficulty finding things at home or at work?

_____11. How often are you distracted by activity or noise around you?

_____12. How often do you leave your seat in meetings or other situations in which you are expected to remain seated?

_____13. How often do you feel restless or fidgety?

_____14. How often do you have difficulty unwinding and relaxing when you have time to yourself?

_____15. How often do you find yourself talking too much when you are in social situations?

_____16. When you’re in a conversation, how often do you find yourself finishing the sentence of the person who was talking?

_____17. How often do you have difficulty waiting your turn in situations when turn taking is required?

_____18. How often do you interrupt others when they are busy?

_____19. When you have a task that requires a lot of thought, how often do you avoid or delay getting started?


Evaluation:

This tool is only a screening device. If you have ANY 3's or 4's you may wish to have a medical doctor evaluate you. This likely would include reports about you from other people in your daily life. If your total score was more than 38, it indicates that further diagnosis may be appropriate. 

The Value of Screening for Adults With ADHD: 

Research suggests that the symptoms of ADHD can persist into adulthood, having a significant impact on the relationships, careers, and even the personal safety of those who may suffer from it. Because this disorder is often misunderstood, many people who have it are never diagnosed. Therefore, they do not receive appropriate treatment and never reach their full potential.
